Information for patients

You have been referred to our clinic by one of your physicians/clinicians for Electrodiagnostic consultation and testing (Drs Leblond, Deng and Gumbs)

EMG (Electromyography) and NCS (Nerve conduction studies) are techniques used to assess the function of your muscles and peripheral nerves. These studies will help determine whether symptoms of numbness, tingling, pain, and weakness are caused by a nerve or muscle disease. The tests are well tolerated and are fully covered by Provincial Health Care programs

Test results will be reviewed with you and a complete consultation report will be sent to your referring physician within a few days or your assessment

Confirm your appointment

Arrive 15 minutes early

Bring a pair of shorts and a T-shirt

Do not apply lotion of any kind to your arms or legs

Complete the Intake Questionnaire form

and return it via email at moa@catalystforhealth.ca

if you prefer, bring it with you on your scheduled visit

and if you can't complete it prior to your appointment, please come at least 20 minutes before your scheduled visit time to complete it

Please send your referrals by fax or Accuro EMR

We will send you a detailed consultation report by fax typically within 1-7 days of your patient visit

Critical results will be reported immediately

If you have an URGENT referral please call the clinic and we would be happy to discuss and help expedite the assessment
